Dr. Shadae Booker-Shelton

Dr. Shadae Booker-Shelton is a board-certified Nurse Practitioner specializing in integrated primary care and mental health services. She provides comprehensive, evidence-based care to patients across the lifespan; with a strong emphasis on accessibility, care coordination, and whole-person wellness.


Dr. Booker-Shelton earned her Doctor of Nursing Practice degree from Northern Kentucky University in 2022 and obtained her Psychiatric Mental Health Nurse Practitioner certification in August 2025. She completed her Master of Science in Nursing as a Family Nurse Practitioner at Governors State University in 2019, a Bachelor of Science in Nursing from Purdue University in 2015, and a Bachelor of Business Administration from DeVry University in 2007. Her education background informs of a systems-based approach to healthcare delivery, emphasizing operational excellence, sustainability, and patient-centered care design.


Her clinical background spans a wide range of practice settings, including primary care, psychiatric medication management, integrated care, geriatric care, acute and chronic kidney disease, dialysis, medical-surgical nursing, women’s health/oncology care for pre- and post-operative inpatient patients. She brings extensive experience managing complex chronic conditions, coordinating interdisciplinary care, and supporting patients across the continuum of care.


As co-owner of A & S Divine Healthcare, Dr. Booker-Shelton is dedicated to advancing an integrated care model that addresses both physical and mental health needs within a single clinical home. Her practice philosophy emphasizes compassionate, respectful care, patient education, and shared decision-making to support improved health outcomes.


In addition to her clinical work, Dr. Booker-Shelton is an experienced nurse educator and serves as a professor and preceptor for nursing and nurse practitioner students. She is an active member of the American Nurses Association (ANA) and is involved in community-focused initiatives aimed at reducing barriers to care, addressing health disparities, and promoting wellness across the lifespan. Through her leadership at ASDHC, she remains committed to improving access to high-quality healthcare for individuals, families, and underserved communities.
